Prenatal Care Coordination (PNCC)

The Prenatal Care Coordination (PNCC) program is designed to address the needs and concerns of expectant mothers. The program educates pregnant women on living skills including good eating habits and health practices. The goal of the program is to improve birth outcomes by offering mothers the medical, supportive and social services needed to have a healthy baby. Prenatal care is especially critical to reduce the risk of complications to the mother and infant.

Outreach physicians provide routine outpatient prenatal and post-partum care, collaborating with our hospital partners who provide support during your delivery.

Available Services:

  • Assistance with baby items such as clothing, formula, diapers etc.

  • Assistance with transportation

  • Locating housing, adult education resources

  • Assistance with food program application

  • Referrals to local community agencies

  • Employment location assistance

Services are available during pregnancy and for the following 60 days after birth. A care plan is developed centered around the expectant mothers’ individual needs and provides support and resources needed for a healthy baby.

What happens once the baby is born?

Becoming a parent for the first time is amazing but it does come with challenges. It can be very exciting but can also be stressful. The Child Care Coordination (CCC) program supports and addresses the concerns that come with being a new mother.

Child Care Coordination (CCC)

The Child Care Coordination (CCC) program is available for mothers with newborns that may need encouragement, motivation and support. The support can help relieve some of the stress that new mother’s experience. The program teaches women to effectively care for their newborns.

A case manager will assist you in a number of ways:

  • Encourage the mother to choose a primary health care provider for the baby

  • Inform the mother on the importance of immunizations and regular well-child check ups

  • Promote positive parenting skills

  • Help with community service referrals including day care options

  • Assistance with food, diapers, wipes, clothing and blankets

The Child Care Coordination program will begin assisting the parent 60 days after having the baby and will continue this service in person or over the phone up to the child’s 7th birthday. What are the PNCC program
requirements?

The PNCC program is for women who are currently pregnant or who have delivered 8 weeks or less. Everyone will be screened for services and coverage if uninsured or underinsured. Benefit specialist may assist if necessary. There are no age restrictions for entering the program.

What happens once I enroll in the PNCC program?

A case manager will meet with each new mother and conduct a comprehensive assessment. The assessment includes information on current and past pregnancies, health history, eating habits, relationships, stressors and worries. The case manager develops an individual care plan or the term of the pregnancy and up to 60 days after delivery
