Primary Health Care can address the majority of your healthcare needs throughout your life. It includes services related to prevention, treatment and rehabilitation. It also includes care that provides relief from the symptoms and stress associated with serious illness. Most health screenings can also be conducted by primary care providers.

Treatment for Non-Life-Threatening Illnesses and Conditions
Outreach physicians and nurse practitioners can address minor illnesses and conditions, and will prescribe medications, as necessary, to help patients recover optimal health.
Chronic Disease Management — Chronic health conditions, such as diabetes and hypertension (high blood pressure) can lead to life-threatening events if not properly managed. Outreach providers not only provide effective treatment, but they will work with you to help you learn how to manage your condition, with a focus on broader determinants of health, such as diet, exercise and stress management.
Certified Diabetic Education — Whether you were recently diagnosed or have had diabetes for a long time, Outreach’s Certified Diabetic Educator can help you develop a plan to stay healthy and give you the tools and support needed to make that plan a regular part of your life. Certified Diabetic Education is a recognized part of your care and is covered by Medicare and most health insurance plans.
Immunizations and Health Screenings — Getting recommended immunizations and undergoing routine health screenings are important for avoiding and/or detecting more serious health concerns. Vaccines provide important protection against the flu, pneumonia, shingles, cervical cancer and other adult illnesses. Regular screenings for cervical cancer, colorectal cancer and breast cancer improve early detection and health outcomes for people who experience those diseases.
Coordinated Access to Specialty Care — For uninsured patients, Outreach has an agreement with Froedtert and the Medical College of Wisconsin to provide specialty care to patients in need.
