Patient-Centered Medical Home (PCMH):

Our PCMH programs provide a team approach to delivering coordinated healthcare services that focus on the whole person. Our care team includes doctors, nurse practitioners, nurses, medical assistants, integrated behavioral health coordinators, patient services representatives, care coordinators, pharmacists, and more. You can expect the following from your team members:

  • We will work closely with you and take our time to answer any questions you may have regarding your health and wellness plan.

  • We will share ways and provide resources for you to stay healthy.

  • We will remind you of appointment dates and times.

  • We will schedule your appointments to minimize your wait time in our clinics.

  • We will provide you assistance signing up for MyChart.

  • We will provide you assistance if needed for enrollment and homeless programs and services.

How to Make the Most of Your Appointment!

To make the most of your doctor's appointment, we recommend following our new patient checklist:

Write out your questions before your appointment.

It's easy to forget if you try to rely on your memory alone, and sometimes your nerves can make it harder to remember. Writing questions down helps you prioritize your thoughts, so you'll know what to ask first.


Bring a list of your current medications.

Including herbs, vitamins, and other dietary supplements – those count as medication too.


Bring your Identification

Bring your insurance card and personal identification for your appointment.


Be on time for your appointment.

Arrive 15 to 20 minutes early for your appointment. Doing so will give you plenty of time to check in and monitor your vitals & to Connect with your Provider


Try to relax.

It's natural to be nervous or anxious when visiting a healthcare provider. When you arrive or sign on, take a few deep breaths and remember that your provider is on your get-well and stay-well team.


Be honest with your provider

Many medical conditions & family health history are uncomfortable to discuss, especially in-person. Allow yourself to be comfortable sharing honestly with your provider.
