For more than 40 years, Outreach Community Health Centers has been a trusted source of high-quality care for individuals and families across Milwaukee. Today, Outreach is amplifying access through a growing network of care that meets patients where they are, including our new Racine location, strong partnership with C.H. Mason Health Clinic, and a Mobile Health Unit that actively engages neighborhoods throughout the city.
Our primary care clinic provides comprehensive outpatient services for the entire family, with a continued commitment to serving those without health insurance and reducing barriers to care.
Our integrated continuum of care includes primary care, behavioral health, chronic disease management, dental services, vaccinations, prenatal care and care coordination, on-site lab services, an in-house pharmacy, OB/GYN, podiatry, telehealth, well-child visits, and more. In every location and community touchpoint, Outreach remains committed to advancing health equity and delivering care with dignity to strengthening families, and building healthier communities across Milwaukee and Racine.
What do I need to know for my first appointment?
Please bring any/all insurance cards and photo identification with you. If you are uninsured, please bring your last two pay stubs, as Outreach Community Health Centers offers an income-based sliding fee scale related to income and household size for payment of services.
Please arrive 10-15 minutes prior to your schedule appointment time.

Same Day Appointments Now Available at Outreach
Primary Care walk-In appointments are welcome based on provider availability. Walk-in patients are seen on a first-come, first-served basis. While we do our best to accommodate, we cannot guarantee same-day service. Same-day appointments may be available on a limited basis.
To be eligible for a walk in appointment:
Patients must arrive by 5:00 PM Monday–Thursday and must arrive by 4:00 PM on Friday.
If you are experiencing a medical emergency, please go to the nearest emergency room for immediate care.
