I understand that submitting this form constitutes a commitment to the sponsorship level selected above.

I understand that sponsorship benefits are secured upon receipt of the completed commitment and payment, subject to applicable deadlines.

I understand that Outreach Community Health Centers reserves the right to approve exhibitors, promotional materials, giveaways and activities to ensure alignment with the purpose and safety of the Community Health & Resource Fair.

I understand that booth assignments are determined by Outreach Community Health Centers and are subject to availability.
